Ramipril, developed by Hoechst in Germany and first marketed in France in 1989, is a precursor drug that is absorbed from the gastrointestinal tract and hydrolyzed in the liver to produce the active angiotensin-converting enzyme (ACE) inhibitor ramiprilat, a potent and prolonged-acting ACE inhibitor. Administration of ramipril results in an increase in plasma renin activity and a decrease in angiotensin II and aldosterone plasma concentrations. Because of the decrease in angiotensin II, ACE inhibitors lead to peripheral vasodilation and decreased vascular resistance, resulting in beneficial hemodynamic effects. It is used clinically in primary hypertension, congestive heart failure, renal hypertension and in those with symptoms of congestive heart failure that occur within the first few days after an acute infarction.
