Simvastatin is synthesized from the fermentation product of Aspergillus terreus, itself an inactive lactone, whose active structure is catalyzed by an inhibitor of 3 hydroxy-3 methylglutaryl coenzyme A (HMG-CoA) reductase. first marketed in the UK in 1988. Clinical results confirm that it can reduce the levels of total cholesterol, low-density lipoprotein cholesterol (LDL-C) and very low-density lipid white cholesterol (VLDL-C). In addition, it can also raise high-density lipoprotein cholesterol (HDL-C) and lower triglycerides. It is indicated for primary hypercholesterolemia and LDL cholesterol, while raising HDL cholesterol and lowering high triglyceride levels in serum.

1. Simvastatin catalyzes the conversion of β-methyl-β-hydroxyglutarate coenzyme A (HMG-CoA) to β-methyl-p-α-dihydroxyglutarate, which is a specific inhibitor of HMG-CoA reductase. It is hydrolyzed to the corresponding β-hydroxy acid after oral administration. It is one of the effective and safe drugs among the current lipid-lowering drugs, which can reduce serum total cholesterol, LDL, very low density lipoprotein, triglyceride, and moderately increase HDL cholesterol.
2. Simvastatin is used clinically in primary hypercholesterolemia, heterozygous familial and non-familial hypercholesterolemia or mixed hyperlipidemia where dietary therapy is ineffective, and has a significant effect on lowering total cholesterol and LDL cholesterol.
