Amiodarone hydrochloride is a III antiarrhythmic drug that prolongs the atrial and ventricular myofibrillar action potential duration (APD), which is caused by blocking the delayed rectifier outward potassium flow (Ik), which can be characterised as a fast delayed rectifier (Ikr) and a slow delayed rectifier (Iks). Amiodarone has a blocking effect on both Ikr and Iks. The normal cardiomyocyte 3-phase repolarisation current consists of a mixture of Ik force and Iks, but the repolarisation current component of Ikr is increased in bradycardia and the repolarisation current component of Iks is increased in tachycardia, so that amiodarone is superior to pure III drugs (selective Ikr blockers), which exhibit use-dependence, i.e. its antiarrhythmic effect is increased when the heart rate is accelerated and its effect on the QT interval is reduced when the heart rate is slowed This results in a greater antiarrhythmic effect and a lower incidence of tip-twisting ventricular tachycardia (TdP).
